Job Title: Data Coordinator

Requirements:  RN, OTA, PTA

Duties:  Data Coordinators place post discharge phone calls to patients that have been in a physical rehabilitation facility, acute care or are enrolled in a study. 

Hours:  This position is per diem and the hours are flexible.  Employees can make their own schedule while meeting their predetermined monthly work commitment.    

Professional Licensure and Registration: Employees who are required by law to possess and maintain a valid license in their discipline. A copy of the current license/registration will be placed in your employee file. Employees who do not produce a current license/registration will not be able to work until the license/registration is produced.

Employee Orientation:  A new job presents new challenges, surroundings, associations and responsibilities. To help you get acquainted, a two day paid orientation will be held prior to any assignment is given.

Job Title:  Nurse Manager

Nurse Manager Job Responsibilities:
Promotes the reliability and integrity of follow-up data collection by IT HealthTrack, (ITH) Inc. nursing staff. Provides day-to-day management of nurses who are assessing patients by telephone interviews and performing functional assessments. The successful candidate will be responsible for long-term planning of ITH clinical needs by directing and developing staff,collaborating with management forecasting needs of a growing enterprise.

Nurse Manager Job Duties:

  • Identifies nursing service needs by establishing a personal rapport with data collection nurses and other persons in a position to understand service requirements.
  • Maintains nursing guidelines by writing and updating policies and procedures.
  • Maintains nursing operations by initiating, coordinating, and enforcing program, operational, and personnel policies and procedures.
  • Assures quality of nursing service by developing and interpreting company philosophies and standards; enforcing adherence to the state board of nursing and state nurse practice act requirements and to other governing agency regulations; making or recommending adjustments.
  • Maintains nursing staff by recruiting, selecting, orienting, and training nurses.
  • Maintains nursing staff job results by coaching, counseling, and disciplining employees; planning, monitoring, and appraising job results.
  • Provides information to nursing staff by answering questions and requests.
  • Resolves patient needs by utilizing multidisciplinary team strategies.
  • Maintains patient confidence and protects operations by monitoring confidential information processing.
  • Maintains a cooperative relationship among nursing staff and management by communicating information; responding to requests; building rapport; participating in team problem-solving methods.
  • Contributes to team effort by accomplishing related results as needed

Specific Tasks for ITH

  • Conducts data review
  • Supervise nursing staff at 3 months and annual reviews
  • Supervision of new nurses, review phone records, adherence to policies
  • Deal with issues from nursing staff throughout the week such as questions and concerns
  • Complete monthly assignment for each nurse to conduct follow-up
  • Ensure pending records get out to nurses throughout the month.
  • Conduct follow-up interviews as needed

Nurse Manager Skills and Qualifications:

  • Bachelor’s Degree in a clinical discipline (RN, OT, PT, SLP) at Minimum
  • Supervisor or management experience of at least 5 years
  • Intermediate skills with Microsoft Excel, Access, Word and Outlook
  • Experience with hiring, training and mentoring clinical staff
  • Intimate knowledge of HIPAA requirements and able to pass a certification
  • Knowledge of research methods is a plus
  • Able to pass a background check
  • A successful candidate for this position will be available to work 20 hours a week, Monday are required, the rest of the schedule can be flexible
